CARING FOR YOUR STOMA

A stoms ( the Greek word for "mouth") is shiny , wet and red in color similar to the inside lining of your mouth . it will usually have a round or oval shape , varying in size . After surgery , the stoma may appear swollen . It's will go down and gradually shrink in size after a few months .
A stoma doesn't have nerve endings, therefore , it doesn't transmit pain or other sensations. However ,it is rich in blood vessels and may bleed sightly if irritate or rubbed. THIS IS NORMAL!!!
The skin surrounding stoma , called a peristomal skin . Must be protected from direct contact with the discharge . This discharge is potentially irritating . It's should be cleansed gently. Water will not enter the stoma because of its gentle outward contractions, so it;s not necessary to cover the stoma while bathing or showering .

What is an ostomy?

• Stoma = Greek meaning mouth or orifice

• Ostomy = a man-made stoma, usually referred to orifice to the surface of body, and characterised by the lack of control of discharge

HISTORY OF THE OSTOMY

Nature: Incarcerated hernia> outwards rupture > stoma

War: Stabs, cuts to the abdomen > Ostomy

1710: Dr. Littre (France) described a method of creating a stoma

1776: Dr. Pillore (France) performed the first Ostomy

1878: Dr. Maydl (Germany) performed the first proctosigmoidectomy

• 1950:  Ostomy belt with a glass bowl, rubber bag, rubber cap
• 1957:  First disposable colostomy bag of plastic is made - nurse Elise Sørensen contrived the idea.

After colostomy surgery , your stool will pass through your stoma , the opening on your abdomen and empty into pouch .

You will no longer have voluntary control over bowel movements.

Why i'm trying to make a blog?

Actually i'm still in learning process. Try to be as good as i can as a leader.. as an Enterostomal Therapy Nurse .. As a local Preceptor .. Make a blog is not just to share our daliy activity but if able to share as much as i can about stoma care . See!

 My patient with an ileostomy

 An Ileostomy is a surgically created opening intio small intestine through  the abdomen . 

It takes the role of eliminating waste when a number of situations make it necessary to bypass the rectum or colon . 

An ileostomy was constructed from the ileum

An external drainable collection  pouch that has been modified by this patient . VERY creative !!

Special considerations:

• Output contains active digestive enzymes that are corrosive to skin.
• Increase fluids intake (lack of colon function) to avoid dehydration .
• Chew food well as stoma cannot stretch to pass large pieces of undigested food
• Avoid high fibrous foods which can cause blockages in the ileum e.g. nuts , dried fruits , celery, corn , pineaplle, raw carrots. popcorn.
• Medications:( Inform dovtor/physician of special situation .

 Clip was changed to bottle and cap. Nice ,isn't it?

Stool consistency :

~fluids/semi - formed stools with digestive enzymes - normal output 500-800ml/day.

Pouch:

~Drainable one or two-piece with clip.
